- Athletic Clearance: Fill out the online athletic clearance form on the Home-Campus.
- Physical: You must have an up-to-date physical. If your doctor can’t see you in time, consider visiting a Dr. Mikes location for a walk-in appointment. The physical form and additional information are available on the Granite Hills Athletic website.
- Clearance Card: Once your online form and physical are complete, get a clearance card from the athletic office.
