*Emails will only be accepted from the parent/guardian email listed on Aeries or HomeCampus.
*By sending this email, it serves as your electronic signature for the paper form below.
*Parent/guardian is still required to show valid photo ID to take student home from the sporting event.
*Students will only be released to the Parent/Guardian. Emergency contacts cannot pick up the student with this form.
